(1) applies on a form provided by the board;

Terms Used In Alaska Statutes 08.65.050

  • Evidence: Information presented in testimony or in documents that is used to persuade the fact finder (judge or jury) to decide the case for one side or the other.
  • person: includes a corporation, company, partnership, firm, association, organization, business trust, or society, as well as a natural person. See Alaska Statutes 01.10.060
(2) pays the fees required under Alaska Stat. § 08.65.100;
(3) furnishes evidence satisfactory to the board that the person has not engaged in conduct that is a ground for imposing disciplinary sanctions under Alaska Stat. § 08.65.110;
(4) furnishes evidence satisfactory to the board that the person has completed a course of study and supervised clinical experience; the study and experience must be of at least one year’s duration;
(5) successfully completes the examination required by the board.
