35-14-121. Forms. (1) (a) The secretary of state may prescribe and furnish on request forms for:

- Corporation: A legal entity owned by the holders of shares of stock that have been issued, and that can own, receive, and transfer property, and carry on business in its own name.
- State: when applied to the different parts of the United States, includes the District of Columbia and the territories. See Montana Code 1-1-201
(i)an application for a certificate of existence or certificate of registration;
(ii)a foreign corporation‘s registration statement;
(iii)a foreign corporation’s statement of withdrawal;
(iv)a foreign corporation’s transfer of registration statement; and
(v)the annual report.
(b)If the secretary of state requires, use of these forms is mandatory.
(2)The secretary of state may prescribe and furnish on request forms for other documents required or permitted to be filed by this chapter, but their use is not mandatory.
