Applications for designation of a development as an FQD shall be made on Form RPM-BSP-ADA-1, “”Development of Regional Impact Application for Development Approval,”” effective date: 11/90, as incorporated by reference in Rule 9J-2.010, Florida Administrative Code, and in addition thereto those items specified in Rule 9J-28.008, Florida Administrative Code, “”General Requirements,”” and Rule 9J-28.009, Florida Administrative Code, “”Requirements for Designation as a Florida Quality Development.”” The form may be obtained by submitting a request to: State of Florida Department of Community Affairs, Division of Community Planning, 2555 Shumard Oak Boulevard, Sadowski Building, Tallahassee, Florida 32399-2100.
