(1) In accordance with Florida Statutes § 456.073(3), and Florida Statutes § 120.695, when a complaint is received, the Department shall issue a notice of noncompliance as a first response to a minor violation of a rule. Failure of a licensee to take action in correcting the violation within the 15 days after notice may result in either the issuance of a citation when appropriate or the institution of regular disciplinary proceedings. “”Minor violations”” as used in sections 456.073(3), and 120.695, F.S., are defined as follows:
(2) Failure to notify of a change of address within 60 days as required by Fl. Admin. Code R. 64B3-1.006
(3) Attempting to pay any of the fees set forth in chapter 64B3-9, F.A.C., by tendering a check payable to the Board of Clinical Laboratory Personnel or to the Department of Health that is dishonored by the institution upon which it is drawn.
(4) Failure to provide documentation of the national certifications of training program faculty as required by subsection 64B3-3.001(6) and Fl. Admin. Code R. 64B3-3.002(2)(b)
