(1) Any licensee may elect at the time of license renewal to place the license into inactive status by paying the inactive status fee of subsection 64B13-6.001(3), F.A.C.

Terms Used In Florida Regulations 64B13-11.001

  • Jurisdiction: (1) The legal authority of a court to hear and decide a case. Concurrent jurisdiction exists when two courts have simultaneous responsibility for the same case. (2) The geographic area over which the court has authority to decide cases.
    (2) An inactive status licensee may change to active status at any time provided the licensee meets the requirements of Fl. Admin. Code R. 64B13-5.001, for each biennium for which their license has been inactive:
    (a) Discloses any disciplinary action that has been taken against any license to practice optometry the practitioner possessed in any jurisdiction during the time period in which the Florida license was inactive;
    (b) Pays the reactivation fee of subsection 64B13-6.001(3), F.A.C.; and,
    (c) If applicable, the change of status fee in subsection 64B13-6.001(4), F.A.C.
    (3) An inactive status licensee whose license has been in inactive status for more than two consecutive biennial licensure cycles and who applies for active status may change to active status at any time provided the licensee meets the following requirements:
    (a) Meets the continuing education requirements of Fl. Admin. Code R. 64B13-5.001, for each biennium for which their license has been inactive;
    (b) Takes and obtains a passing score on the laws and rules examination;
    (c) Discloses any disciplinary action that has been taken against any license to practice optometry the practitioner possessed in any jurisdiction during the time period in which the Florida license was inactive;
    (d) Pays the reactivation fee of subsection 64B13-6.001(3), F.A.C.; and,
    (e) If the request to change licensure status is made at any time other than at the beginning of a licensure cycle, pays the change of status fee of subsection 64B13-6.001(4), F.A.C.; and,
    (4) Pursuant to Section 463.015(1)(a), F.S., it is unlawful to practice optometry with an inactive license.
