Applicants for CPC shall submit an application using Form DH5059-MQA (eff. 08/20), “”Application for Pharmacist Collaborative Practice Certification”” that is hereby incorporated by reference and available at http://www.flrules.org/Gateway/reference.asp?No=Ref-12225 or http://floridapharmacy.gov Applicants for certification shall meet and comply with all requirements in Florida Statutes § 465.1865
