(1) All paternity judgment orders issued or modified by a court on or after July 15,
1992, shall include, but not be limited to, the following information about the father, if the information is known at the time the order is set:

Terms Used In Kentucky Statutes 213.049

  • Foreign: when applied to a corporation, partnership, limited partnership, business trust, statutory trust, or limited liability company, includes all those incorporated or formed by authority of any other state. See Kentucky Statutes 446.010
  • Jurisdiction: (1) The legal authority of a court to hear and decide a case. Concurrent jurisdiction exists when two courts have simultaneous responsibility for the same case. (2) The geographic area over which the court has authority to decide cases.
  • Month: means calendar month. See Kentucky Statutes 446.010
  • State: when applied to a part of the United States, includes territories, outlying possessions, and the District of Columbia. See Kentucky Statutes 446.010
  • Vital statistics: means the data derived from certificates and reports of birth, death, stillbirth, abortion, marriage, dissolution of marriage, and related reports. See Kentucky Statutes 213.011

(a) Full name;
(b) Social Security number;
(c) Age at time of the child’s birth;
(d) Place of birth as to the state or foreign country; and
(e) Current address.
(2) The order shall be transmitted by the Circuit Court clerk of the court with jurisdiction to the state registrar of vital statistics no later than the fifteenth day of each calendar month.
