Texas Election Code 15.053 – Response to Confirmation Notice
(a) The voter shall submit to the registrar a written, signed response to the notice that confirms the voter’s current residence. The response must contain:
(1) all of the information that a person must include in an application to register to vote under § 13.002;
(2) a sworn affirmation of the voter’s current residence as defined by § 1.015; and
(3) if the voter’s residence address is a commercial post office box or similar location that does not correspond to a residence, evidence of the voter’s residence address as required by § 15.054 or an indication that the voter is exempt from those requirements.
(b) The voter shall use an official confirmation notice response form for the response unless the voter does not have possession of the official form at the time of making the response.

- Evidence: Information presented in testimony or in documents that is used to persuade the fact finder (judge or jury) to decide the case for one side or the other.
- Person: includes corporation, organization, government or governmental subdivision or agency, business trust, estate, trust, partnership, association, and any other legal entity. See Texas Government Code 311.005
- Residence address: means the street address and any apartment number, or the address at which mail is received if the residence has no address, and the city, state, and zip code that correspond to a person's residence. See Texas Election Code 1.005
- Signed: includes any symbol executed or adopted by a person with present intention to authenticate a writing. See Texas Government Code 311.005
- sworn: includes affirm or affirmed. See Texas Government Code 312.011
- Written: includes any representation of words, letters, symbols, or figures. See Texas Government Code 311.005
(c) The registrar shall act on the response in accordance with § 13.072 and retain the response on file with the voter’s registration application.
