The certificate of competency issued under section 4104.07 of the Revised Code or the commission provided for in section 4104.08 of the Revised Code may be revoked by the superintendent of industrial compliance for the incompetence of the holder thereof, or for willful falsification of any matter or statement contained in the holder’s application or in a report of any inspection in accordance with Chapter 119 of the Revised Code. If a certificate or commission is lost or destroyed, a new certificate or commission shall be issued in its place without another examination.
