Ohio Code 4713.641 – Complaints
Any student or former student of a school of cosmetology licensed under division (A) of section 4713.44 of the Revised Code may file a complaint with the state cosmetology and barber board alleging that the school has violated division (A) of section 4713.64 of the Revised Code. The complaint shall be in writing and signed by the individual bringing the complaint. Upon receiving a complaint, the board shall initiate a preliminary investigation to determine whether it is probable that a violation was committed. If the board determines after preliminary investigation that it is not probable that a violation was committed, the board shall notify the individual who filed the complaint of the board’s findings and that the board will not issue a formal complaint in the matter. If the board determines after a preliminary investigation that it is probable that a violation was committed, the board shall proceed against the school pursuant to the board’s authority under section 4713.64 of the Revised Code and in accordance with the hearing and notice requirements prescribed in Chapter 119 of the Revised Code.

- Complaint: A written statement by the plaintiff stating the wrongs allegedly committed by the defendant.
- Cosmetology: means the art or practice of embellishment, cleansing, beautification, and styling of hair, wigs, postiches, face, body, or nails. See Ohio Code 4713.01
- in writing: includes any representation of words, letters, symbols, or figures; this provision does not affect any law relating to signatures. See Ohio Code 1.59
- state: means the state of Ohio. See Ohio Code 1.59
- Student: means an individual, other than an apprentice instructor, who is engaged in learning or acquiring knowledge of the practice of a branch of cosmetology at a school of cosmetology. See Ohio Code 4713.01
