(a) The Department of Public Health shall prescribe forms for an acknowledgment of parentage. Such forms shall include the minimum requirements specified by the Secretary of the United States Department of Health and Human Services, contained in 45 C.F.R. § 303.5, as amended from time to time, and shall be in compliance with the provisions of public act 21-15*. Any acknowledgment or rescission executed in accordance with this subsection shall be filed in the parentage registry established and maintained by the Department of Public Health under section 19a-42a.

- Rescission: The cancellation of budget authority previously provided by Congress. The Impoundment Control Act of 1974 specifies that the President may propose to Congress that funds be rescinded. If both Houses have not approved a rescission proposal (by passing legislation) within 45 days of continuous session, any funds being withheld must be made available for obligation.
(b) A valid acknowledgment of parentage is not affected by a later modification of the form under subsection (a) of this section.
