(1) A person who finds lost or mislaid property and delivers it to a local law enforcement agency shall sign a statement included in a form provided by the agency, stating:

See Utah Code 77-11d-101 - Person: means :(24)(a) an individual;(24)(b) an association;(24)(c) an institution;(24)(d) a corporation;(24)(e) a company;(24)(f) a trust;(24)(g) a limited liability company;(24)(h) a partnership;(24)(i) a political subdivision;(24)(j) a government office, department, division, bureau, or other body of government; and(24)(k) any other organization or entity. - Property: includes both real and personal property. See Utah Code 68-3-12.5
(1)(a) the manner in which the property came into the person’s possession, including the time, date, and place;(1)(b) that the person does not know who owns the property;(1)(c) that, to the person’s knowledge, the property was not stolen;(1)(d) that the person’s possession of the property is not unlawful; and(1)(e) any information the person is aware of which could lead to a determination of the owner.
(2) Additional information may be requested by the agency receiving the property, as necessary.
