All complaints concerning a licensee’s business or professional practice shall be received by either the licensing board or the department of health. Each complaint received shall be logged, recording at a minimum the following information:
(1) Licensee’s name;
(2) Name of the complaining party;
(3) Date of complaint;
(4) Brief statement of complaint; and
(5) Disposition.

Terms Used In Rhode Island General Laws 5-69-11
- board: means the board of licensing for chemical dependency professionals. See Rhode Island General Laws 5-69-2
- Complaint: A written statement by the plaintiff stating the wrongs allegedly committed by the defendant.
- Department: means the Rhode Island department of health. See Rhode Island General Laws 5-69-2
