covered individual: means an individual who is entitled to coverage under a policy of accident and sickness insurance. See Indiana Code 27-8-34-1
health care services: has the meaning set forth in IC 27-8-11-1. See Indiana Code 27-8-34-2
policy: means a policy of accident and sickness insurance (as defined in IC 27-8-5-1). See Indiana Code 27-8-34-3
provider: has the meaning set forth in IC 27-8-11-1. See Indiana Code 27-8-34-4
telehealth services: means health care services delivered by use of technology allowed under Indiana Code 27-8-34-5
