Sec. 4. The division shall notify in writing the person and the provider of the following:
(2) The reasons for a denial of eligibility.

- Appeal: A request made after a trial, asking another court (usually the court of appeals) to decide whether the trial was conducted properly. To make such a request is "to appeal" or "to take an appeal." One who appeals is called the appellant.
- in writing: include printing, lithographing, or other mode of representing words and letters. See Indiana Code 1-1-4-5
(3) That either party has the right to appeal the decision.
