Sec. 3.1. (a) To apply for a health facility administrator license or a residential care administrator license, an applicant shall do the following:
(2) Provide evidence that the applicant meets the requirements of this chapter.
(1) Electronically submit an application to the board on a form prescribed by the board.
Terms Used In Indiana Code 25-19-1-3.1
- agency: refers to the Indiana professional licensing agency. See Indiana Code 25-19-1-1
- board: refers to the Indiana state board of health facility administrators. See Indiana Code 25-19-1-1
- Evidence: Information presented in testimony or in documents that is used to persuade the fact finder (judge or jury) to decide the case for one side or the other.
- health facility administrator: means a natural person who administers, manages, supervises, or is in general administrative charge of a licensed health facility whether such individual has an ownership interest in the health facility and whether the person's functions and duties are shared with one (1) or more individuals. - residential care administrator: means an individual who:

(b) If the agency is unable to resolve a discrepancy or issue of concern regarding an individual’s application under this section, the board may require the applicant to appear before the board if the board determines the application:
(1) is incomplete; or
(2) lacks sufficient evidence that the applicant meets the requirements of this chapter.
