Indiana Code 25-26-13-20. Applications for pharmacy permits
(1) the name and occupation of the persons desiring the permit;

- Board: means the Indiana board of pharmacy. See Indiana Code 25-26-13-2
- Person: means any individual, partnership, copartnership, firm, company, corporation, association, joint stock company, trust, estate, or municipality, or a legal representative or agent, unless this chapter expressly provides otherwise. See Indiana Code 25-26-13-2
- Pharmacy: means any facility, department, or other place where prescriptions are filled or compounded and are sold, dispensed, offered, or displayed for sale and which has as its principal purpose the dispensing of drug and health supplies intended for the general health, welfare, and safety of the public, without placing any other activity on a more important level than the practice of pharmacy. See Indiana Code 25-26-13-2
(3) such other information as the board may require.
(b) If the applicant desires to open, establish, operate, or maintain more than one (1) pharmacy, the applicant must file a separate application for each.
(c) The board shall hold the permit holder responsible and may not discipline or otherwise hold an individual licensed under this chapter responsible for staffing deficiencies of the pharmacy if the individual does not have authority for staffing determinations of the pharmacy.
(d) The board shall grant or deny an application for a permit not later than one hundred twenty (120) days after the application and any additional information required by the board are submitted.
(e) The board may not issue a pharmacy permit to a person who desires to operate the pharmacy out of a residence.
