Sec. 0.7. As used in this chapter, “contract holder” means:

(1) an individual or entity that offers health insurance coverage to its employees or members through a self-funded health benefit plan, including a self-funded health benefit plan that complies with the federal Employee Retirement Income Security Act (ERISA) of 1974 (29 U.S.C. § 1001 et seq.);

Terms Used In Indiana Code 27-1-24.5-0.7

  • health plan: means the following:

    Indiana Code 27-1-24.5-5

  • Insurance: means a contract of insurance or an agreement by which one (1) party, for a consideration, promises to pay money or its equivalent or to do an act valuable to the insured upon the destruction, loss or injury of something in which the other party has a pecuniary interest, or in consideration of a price paid, adequate to the risk, becomes security to the other against loss by certain specified risks; to grant indemnity or security against loss for a consideration.   • pharmacy: means the physical location:

    Indiana Code 27-1-24.5-11

(2) a health plan; or

(3) Medicaid or a managed care organization (as defined in IC 12-7-2-126.9) that provides services to a Medicaid recipient;

that contracts with a pharmacy benefit manager to provide services.
