arrangement: refers to a multiple employer welfare arrangement. See Indiana Code 27-1-34-1
Commissioner: means the "insurance commissioner" of this state. See Indiana Code 27-1-2-3

Sec. 4. An arrangement shall file an annual statement on a form prescribed by the commissioner.
