(a) The commissioner or an authorized agent shall issue a crewmember fishing license under Alaska Stat. § 16.05.480 to each qualified person who files a written application at a place in the state designated by the commissioner, containing the reasonable information required by the commissioner together with the required fee. The commissioner shall require the reporting of the applicant’s social security number on the application. The application shall be simple in form and shall be executed by the applicant under the penalty of unsworn falsification in the second degree.

(b) The Alaska Commercial Fisheries Entry Commission shall issue a vessel license under Alaska Stat. § 16.05.490 to each qualified vessel for which a written application has been filed, at a place in the state designated by the commission, containing the reasonable information required by the commission together with the required fee. The application shall be simple in form and shall be executed by the applicant under the penalty of unsworn falsification in the second degree.

(d) Upon request, the commissioner shall provide a social security number provided under (a) of this section to the child support services agency created in Alaska Stat. § 25.27.010, or the child support enforcement agency of another state, for child support purposes authorized under law.
