A licensed optometrist shall deliver to each patient that makes a payment to the practice, excluding insurance copayments and deductibles, a receipt that contains all of the following information:
(a) His or her name.
Terms Used In California Business and Professions Code 3076
- license: means license, certificate, registration, or other means to engage in a business or profession regulated by this code or referred to in Section 1000 or 3600. See California Business and Professions Code 23.7
- optometrist: means a person who is licensed to practice optometry in this state under the authority of this chapter. See California Business and Professions Code 3003
- place of practice: means any location where optometry is practiced. See California Business and Professions Code 3005
(b) The number of his or her optometrist license.
(c) His or her place of practice.
(d) A description of the goods and services for which the patient is charged and the amount charged.
