Connecticut General Statutes 21a-256 – Labeling of package or container of controlled substances
(a) When a manufacturer sells or dispenses a controlled substance and when a wholesaler sells, dispenses or distributes a controlled substance in a package prepared by him, he shall securely affix to each package in which that substance is contained a label showing in legible English the name and address of the vendor and the quantity, kind and form of controlled substance contained therein and any additional information required under the federal food and drug laws and the state food, drug and cosmetic laws. No person, except a practitioner dispensing a controlled substance under this chapter, shall alter, deface or remove any label so affixed.

- Controlled substance: means a drug, substance, or immediate precursor in schedules I to V, inclusive, of the Connecticut controlled substance scheduling regulations adopted pursuant to section 21a-243. See Connecticut General Statutes 21a-240
- Dentist: means a person authorized by law to practice dentistry in this state. See Connecticut General Statutes 21a-240
- Drug: means (A) substances recognized as drugs in the official United States Pharmacopoeia, official Homeopathic Pharmacopoeia of the United States, or official National Formulary, or any supplement to any of them. See Connecticut General Statutes 21a-240
- Nurse: means a person performing nursing as defined in section 20-87a. See Connecticut General Statutes 21a-240
- Official written order: means an order for controlled substances written on a form provided by the bureau for that purpose under the federal Controlled Substances Act. See Connecticut General Statutes 21a-240
- Person: includes any corporation, limited liability company, association or partnership, or one or more individuals, government or governmental subdivisions or agency, business trust, estate, trust, or any other legal entity. See Connecticut General Statutes 21a-240
- Pharmacist: means a person authorized by law to practice pharmacy pursuant to section 20-590, 20-591, 20-592 or 20-593. See Connecticut General Statutes 21a-240
- Pharmacy: means an establishment licensed pursuant to section 20-594. See Connecticut General Statutes 21a-240
- Physician: means a person authorized by law to practice medicine in this state pursuant to section 20-9. See Connecticut General Statutes 21a-240
- Podiatrist: means a person authorized by law to practice podiatry in this state. See Connecticut General Statutes 21a-240
- Practitioner: means : (A) A physician, dentist, veterinarian, podiatrist, scientific investigator or other person licensed, registered or otherwise permitted to distribute, dispense, conduct research with respect to or to administer a controlled substance in the course of professional practice or research in this state. See Connecticut General Statutes 21a-240
- Prescription: means a written, oral or electronic order for any controlled substance or preparation from a licensed practitioner to a pharmacist for a patient. See Connecticut General Statutes 21a-240
- Sale: is a ny form of delivery which includes barter, exchange or gift, or offer therefor, and each such transaction made by any person whether as principal, proprietor, agent, servant or employee. See Connecticut General Statutes 21a-240
- State: when applied to a part of the United States, includes any state, district, commonwealth, territory or insular possession thereof, and any area subject to the legal authority of the United States of America. See Connecticut General Statutes 21a-240
- Veterinarian: means a person authorized by law to practice veterinary medicine in this state. See Connecticut General Statutes 21a-240
- Wholesaler: means a distributor or a person who supplies controlled substances that the person personally has not produced or prepared to registrants. See Connecticut General Statutes 21a-240
(b) When a pharmacist sells or dispenses any controlled substance on prescription issued by a physician, advanced practice registered nurse, physician assistant, podiatrist, dentist or veterinarian, the pharmacist shall affix, to the container in which such substance is sold or dispensed, a label showing the name and address of the pharmacy for which the pharmacist is lawfully acting, the full name of the patient, or, if the patient is an animal, the name of the owner of the animal and the species of the animal, the last name of the physician, advanced practice registered nurse, physician assistant, podiatrist, dentist or veterinarian by whom the prescription was written, such directions as may be stated on the prescription, the serial number of the prescription, the date of filling or refilling and any cautionary statement in such prescription as may be required by law.
(c) When aqueous or oleaginous preparations are sold under subsection (c) of section 21a-250, a label shall be affixed to the container containing the preparation which bears the name, address and BNDD numbers of the vendor and vendee, the date of sale, the kind and quantity of substance sold and the serial number of the official written order. No person shall alter, deface or remove any label affixed pursuant to subsection (b) or this subsection.
