A petition for hearing pursuant to Florida Statutes § 112.322(2), shall contain a statement of the public office or employment held by Petitioner; a statement of the specific allegations made against Petitioner which form the basis of the petition, when and by whom they were made; and a statement of the facts and circumstances which make the allegations of such gravity as to affect the general welfare of the state and the ability of Petitioner effectively to discharge the duties of the office or employment.
