(1) A State of Florida Well Completion Report, DEP Form 62-532.900(2), (October 7, 2010), which is incorporated by reference in Fl. Admin. Code R. 40C-3.036(9)(b), is required for the construction, repair or abandonment of all wells regardless of whether a permit is required under Fl. Admin. Code R. 40C-3.041 Well completion reports shall be completed and filed at a District office identified in Fl. Admin. Code R. 40C-3.101, at the District’s website floridaswater.com, or with the entity to which the authority to issue a permit has been delegated, as identified in Fl. Admin. Code R. 40C-3.035, by the contractor within 30 days of the completion of the work.

    (a) Well completion reports for sites controlled by Fl. Admin. Code Chapter 62-761, may include all monitoring wells for the same site on a single form.
    (b) Computer generated completion reports developed by the contractor may be used in place of District supplied forms if these reports have been approved by the District prior to use.
    (2) The water well contractor shall keep or cause to be kept by a person in his employ a field log. Such records shall be available for inspection by the District at the site during all times when actual work is in progress.
    (3) If no work is performed or if the well is not completed, a report shall be filed by the contractor within thirty days of the expiration of the permit stating that no well construction was performed or completed under the permit.
