(1) Failure to report as required in this section in whole or in part is subject to administrative fines as provided in Florida Statutes § 408.08(5), unless the health insurer has been granted an extension of up to 30 days by the Agency for Health Care Administration for reasons of extraordinary or hardship circumstances such as a natural disaster or emergency event impacting the health insurer or the collection of data required in this section. Health insurers must request the extension, in writing, prior to the due dates specified in Rules 59B-14.004 and 59B-14.005, F.A.C.
