A freight forwarder permit is required for any person that engages in the distribution of a prescription drug as a freight forwarder unless the person is a common carrier. A person, prior to engaging in activity for which a freight forwarder permit is required, must file an application on form number DBPR-DDC-225, Application for Permit as a Freight Forwarder, effective April 2016, adopted and incorporated herein by reference and comply with all the requirements for permitting in Florida Statutes Chapter 499, and Title 61N, F.A.C.
