A Retail Pharmacy Drug Wholesale Distributor Permit is required for any retail pharmacy as defined in Florida Statutes § 499.003(46), that is engaged in wholesale distribution of prescription drugs within this state under the conditions enumerated in Section 499.01(2)(g), F.S.
An applicant wishing to obtain a Retail Pharmacy Drug Wholesale Distributor Permit must file an application on form number DBPR-DDC-218, Application for Permit as a Retail Pharmacy Drug Wholesale Distributor, effective November 2019, incorporated herein by reference, http://www.flrules.org/Gateway/reference.asp?No=Ref-11584, together with the appropriate fees.
