A Veterinary Prescription Drug Retail Establishment Permit is required for any person that sells veterinary prescription drugs to the public pursuant to the requirements of Sections 499.01(2)(k)1.-7., F.S., but does not include a pharmacy licensed under Florida Statutes Chapter 465 Applicants wishing to obtain a Veterinary Prescription Drug Retail Establishment Permit shall submit a complete application to the Department on form DBPR-DDC-222, Application for Permit as a Veterinary Prescription Drug Retail Establishment, effective November 2019, incorporated herein by reference, https://www.flrules.org/Gateway/reference.asp?No=Ref-11413, together with the appropriate fees.
