(1) Each licensed practitioner and each resident shall conspicuously display a current license issued by the Department and a photograph at his or her practice location.
(3) All unlicensed support personnel shall wear an identification badge that identifies the person as support personnel and meets the requirements set forth in subsection (2), above.

(2) Each licensed practitioner and each resident shall wear an identification badge containing the information required by
Florida Statutes § 468.8095, and a personal photograph of no less than 3/4 inch square in size, that is a minimum size of 2 by 3 inches with the text in a font equal to at least Times New Roman 14 point font or Courier New 12 point font.
(3) All unlicensed support personnel shall wear an identification badge that identifies the person as support personnel and meets the requirements set forth in subsection (2), above.
