A podiatric physician who dispenses medicinal drugs for human consumption for a fee or remuneration of any kind, whether direct or indirect, must register with the Board using the Podiatric Application for Dispensing Practitioner Registration Form DH-MQA 1141 (revised 10/19), hereby adopted and incorporated by reference, that can be obtained from https://www.flrules.org/Gateway/reference.asp?No=Ref-12449 the Board of Podiatric Medicine’s website at http://www.doh.state.fl.us/mqa/podiatry/index.html, and pay a fee of $100.00 at the time of such registration and upon each renewal of licensure.
