(1) The application fee shall be $100.00.
(2) The initial license fee shall be $100.00.
(3) The fee for biennial renewal of an active license shall be $55.00.
(4) The fee for biennial renewal of an inactive license shall be $27.50.
(5) The fee to change licensure status shall be $55.00.
(6) The fee for reactivation of an inactive license shall be $50.00.
(7) A delinquent status license shall pay a delinquency fee of $50.00.
