The Department of Health shall reactivate an inactive license upon receipt of the reactivation application, the biennial licensure fee as prescribed in Fl. Admin. Code R. 64B21-501.003, the reactivation fee as prescribed in Fl. Admin. Code R. 64B21-501.006, and upon the Department’s certification that the licensee has satisfactorily submitted evidence of participation in fifteen (15) hours of approved continuing education for each year the license was on inactive status and thirty (30) hours of approved continuing education for the last biennium in which the licensee held an active license.
