The Board designates the following violations of its practice act as being appropriate for mediation where the subject has an explanation and a differing view from the complainant as to the nature or extent of the violation as provided in Florida Statutes § 456.078, as these violations are economic in nature or can be remedied by the licensee:
(2) Attempting to pay any of the fees set forth in Fl. Admin. Code Chapter 64B3-9, by tendering a check payable to the Board of Clinical Laboratory Personnel or to the Department of Health that is dishonored by the institution upon which it is drawn.
(3) Failure to notify the Department of a change of address within 60 days as required by Fl. Admin. Code R. 64B3-1.006

(1) Failure of the licensee to comply with
Fl. Admin. Code R. 64B3-12.006, or with a Final Order with regard to the time limitation for the payment of fines or costs under Section 483.825(1)(n), F.S.
(2) Attempting to pay any of the fees set forth in Fl. Admin. Code Chapter 64B3-9, by tendering a check payable to the Board of Clinical Laboratory Personnel or to the Department of Health that is dishonored by the institution upon which it is drawn.
