A licensee shall furnish a copy of the records required in Rules 64E-5.643, 64E-5.644, and 64E-5.645, F.A.C., and the output from the teletherapy source expressed as rads (grays) per hour at 1 meter from the source as determined during the full calibration required in Fl. Admin. Code R. 64E-5.641, to the department within 30 days following completion of the action that initiated the record requirement.
