Each governmental contractor and provider, by August 31 of each year, shall submit to the Department of Health information required to prepare the annual report to the Legislature as specified in Florida Statutes § 766.1115, including, participating clinics and organizations, the number of providers, the number of patients, the number of patient visits, and the value of services and donations rendered from July 1 to June 30.
