Form OIR-C1-1208, “”Notification to the Office of Insurance Regulation as a Qualifying Issuer of Donor Annuity Agreements Pursuant to Florida Statutes § 627.481,”” effective 6/20, is hereby incorporated by reference and available at https://www.flrules.org/Gateway/reference.asp?No=Ref-14417. The form may be obtained from https://www.floir.com/iportal.

Terms Used In Florida Regulations 69O-202.008

  • Annuity: A periodic (usually annual) payment of a fixed sum of money for either the life of the recipient or for a fixed number of years. A series of payments under a contract from an insurance company, a trust company, or an individual. Annuity payments are made at regular intervals over a period of more than one full year.
  • Donor: The person who makes a gift.
    (2) Any person subject to Florida Statutes § 627.481, that fails to submit the required notification form is subject to penalty as provided in Florida Statutes § 626.9521
