In conjunction with the requirements of Florida Statutes § 657.065, if the proposed surviving credit union is state chartered, it shall submit a nonrefundable application fee of $500 to OFR, along with an Application for Approval of Merger on Form OFR-U-64, (revised 10/2006), which is hereby incorporated by reference. Form OFR-U-64 may be obtained from the Office of Financial Regulation’s website (www.flofr.gov) or by mail by submitting a request to Office of Financial Regulation, Division of Financial Institutions, 200 East Gaines Street, Tallahassee, Florida 32399-0371. If the proposed surviving credit union is federally chartered, the credit union shall submit a copy of the federal merger application to OFR. The Credit union shall notify OFR, NCUA and other regulatory agencies, as applicable, upon consummation of the merger.
