Whenever at least 100 electors of a specified municipality present a petition to the municipal clerk, asking that the question of the adoption of this Division 18 be submitted to the electors of the municipality the question shall be certified by the clerk and submitted to the electors of the municipality at an election in accordance with the general election law. The question shall be in substantially the following form:

incorporated town) of………… YES
adopt Division 18 of Article 11
of the Illinois Municipal Code ———————–
providing for community nurses in
certain municipalities and permit
a tax of not to exceed .0075% NO
therefor?

If a majority of the votes cast on the question are in favor of the adoption of this Division 18, such division is adopted and in force thereafter in that municipality.
