Sec. 5. The division shall do the following with the disclosure form received from each health facility and housing with services establishment under section 4 of this chapter:
(2) Publish all the disclosure forms before February of each year in a single publication to disseminate to the public upon request.
(1) Make the form available to any individual upon request.
Terms Used In Indiana Code 12-10-5.5-5
- health facility: means a facility that is licensed under or subject to IC 16-28. See Indiana Code 12-10-5.5-2
- housing with services establishment: has the meaning set forth in IC 12-10-15-3. See Indiana Code 12-10-5.5-2.5
- Year: means a calendar year, unless otherwise expressed. See Indiana Code 1-1-4-5
