Sec. 7. (a) Prior to the execution of a continuing care agreement, a provider shall deliver to the contracting party and the prospective resident a copy of the initial disclosure statement and the latest annual disclosure statement.

Terms Used In Indiana Code 23-2-4-7

  • Continuing care agreement: means the following:

    Indiana Code 23-2-4-1

  • Contracting party: means a person or persons who enter into a continuing care agreement with a provider. See Indiana Code 23-2-4-1
  • Provider: means a person that agrees to provide care under a continuing care agreement. See Indiana Code 23-2-4-1
  • Resident: means an individual who is entitled to receive benefits under a continuing care agreement. See Indiana Code 23-2-4-1
     (b) After the execution of a continuing care agreement, a provider shall provide, upon request, a copy of the initial disclosure statement and the latest annual disclosure statement.
