Sec. 9. (a) A putative father who registers under this chapter shall provide to the state department the following:

(1) The putative father’s:

Terms Used In Indiana Code 31-19-5-9

  • Clerk: means the clerk of the court or a person authorized to perform the clerk's duties. See Indiana Code 1-1-4-5
  • Trial: A hearing that takes place when the defendant pleads "not guilty" and witnesses are required to come to court to give evidence.
(A) name;

(B) address at which the putative father may be served with notice of an adoption under Rule 4.1 of the Indiana Rules of Trial Procedure;

(C) Social Security number; and

(D) date of birth.

(2) The mother’s name, including all other names known to the putative father that the mother uses.

(3) Any other information described under section 7 of this chapter that is known to the putative father.

     (b) A clerk of the court shall provide to the state department the notice required to be prepared under IC 31-14-9-0.5.
