Indiana Code 34-59-1-7. “Sworn declaration”

Sec. 7. (a) As used in this chapter, “sworn declaration” means a declaration in a signed record given under oath.

Affidavit: A written statement of facts confirmed by the oath of the party making it, before a notary or officer having authority to administer oaths.
record: means information that is inscribed on a tangible medium or that is stored in an electronic or other medium and is retrievable in perceivable form. See Indiana Code 34-59-1-4

(b) The term includes a sworn statement, verification, certificate, and affidavit.
