  • health care quality indicator data: means information concerning the provision of health care services that may be collected and used to measure and compare quality of health care services. See Indiana Code 16-40-4-1
  • health coverage provider: means any of the following:

  • medical record: means written or printed information possessed by a provider (as defined in IC 16-18-2-295) concerning any diagnosis, treatment, or prognosis of the patient, unless otherwise defined. See Indiana Code 1-1-4-5
  • program: refers to the health care quality indicator data program developed and implemented under sections 4 and 5 of this chapter. See Indiana Code 16-40-4-3
