  • acquisition: means any agreement, arrangement, or activity the consummation of which results in a person acquiring directly or indirectly the control of another person. See Indiana Code 25-1-8.5-1

  • Allegation: something that someone says happened.
  • Amendment: A proposal to alter the text of a pending bill or other measure by striking out some of it, by inserting new language, or both. Before an amendment becomes part of the measure, thelegislature must agree to it.
  • Appeal: A request made after a trial, asking another court (usually the court of appeals) to decide whether the trial was conducted properly. To make such a request is "to appeal" or "to take an appeal." One who appeals is called the appellant.

  • Conviction: A judgement of guilt against a criminal defendant.
  • conviction of concern: means a criminal conviction directly related to the duties and responsibilities of the occupation or profession for which the individual is applying or holds a license or certification as set by the board. See Indiana Code 25-1-1.1-0.7
  • covered individual: means an individual who is entitled to coverage under a health plan. See Indiana Code 25-1-9.1-3
  • covered individual: means an individual who is entitled to be provided health care services according to a health carrier's network plan. See Indiana Code 25-1-9.8-1
  • Damages: Money paid by defendants to successful plaintiffs in civil cases to compensate the plaintiffs for their injuries.
  • deceptive or misleading information: means any information that misrepresents or falsely describes a practitioner's:

  • Dependent: A person dependent for support upon another.
  • dependent: means a natural child, stepchild, or adopted child of a parent who is a member of the armed forces of the United States and who is less than twenty-six (26) years of age and at the time of application under this chapter:

  • direct primary care agreement: means a contract entered into between a primary care provider or an employer of a primary care provider and an individual patient or the patient's legal representative in which the primary care provider or the employer of the primary care provider:

  • distant site: means a site at which a practitioner is located while providing health care services through telehealth. See Indiana Code 25-1-9.5-2
  • Donor: The person who makes a gift.
  • educational degree: means the degree awarded to the practitioner by a college or university in the practitioner's profession, which may be used under the scope of the practitioner's license, including use of its acronym. See Indiana Code 25-1-10.3-3.5
  • electronic transmission: has the meaning set forth in IC 25-26-13-2. See Indiana Code 25-1-9.3-4
  • emergency medical condition: means a medical condition that arises suddenly and unexpectedly and manifests itself by acute symptoms of such severity, including severe pain, that the absence of immediate medical attention could reasonably be expected by a prudent lay person who possesses an average knowledge of health and medicine to:

  • episode of care: means the medical care ordered to be provided for a specific medical procedure, condition, or illness. See Indiana Code 25-1-9.8-1.5
  • Evidence: Information presented in testimony or in documents that is used to persuade the fact finder (judge or jury) to decide the case for one side or the other.
  • eye care professional: means either of the following:

  • Fraud: Intentional deception resulting in injury to another.
  • gender: means the psychological, behavioral, social, and cultural aspects of being male or female. See Indiana Code 25-1-22-1
  • gender reassignment surgery: means any medical or surgical service that seeks to surgically alter or remove healthy physical or anatomical characteristics or features that are typical for the individual's sex, in order to instill or create physiological or anatomical characteristics that resemble a sex different from the individual's sex, including genital gender reassignment surgery or nongenital gender reassignment surgery knowingly performed for the purpose of assisting an individual with a gender transition.   • gender transition: means the process in which an individual shifts from identifying with and living as a gender that corresponds to his or her sex to identifying with and living as a gender different from his or her sex, and may involve social, legal, or physical changes. See Indiana Code 25-1-22-3
  • gender transition hormone therapy: means :

  • gender transition procedures: means any medical or surgical service, including physician's services, practitioner's services, inpatient and outpatient hospital services, or prescribed drugs related to gender transition, that seeks to:

  • Gift: A voluntary transfer or conveyance of property without consideration, or for less than full and adequate consideration based on fair market value.
  • good faith estimate: means a reasonable estimate of the price each provider anticipates charging for an episode of care for nonemergency health care services that:

  • Guardian: A person legally empowered and charged with the duty of taking care of and managing the property of another person who because of age, intellect, or health, is incapable of managing his (her) own affairs.
  • health care entity: means any of the following:

  • health carrier: means an entity:

  • occupational regulation: means a regulation, rule, policy, fee, condition, test, permit, administrative practice, or other provision in which a public agency establishes the personal qualifications necessary to engage in an occupation or profession. See Indiana Code 25-1-16.5-2

  • ophthalmic device: means either of the following:

  • Personal property: All property that is not real property.

  • personal qualifications: means criteria related to an individual's personal background and characteristics, including the following:
