(1) Except for those voters who have been certified as requiring assistance in voting on a permanent or annual basis, any person voting by means of a mail-in absentee ballot or in-person absentee ballot who receives assistance in voting shall be required to complete the voter assistance form required by KRS § 117.255.
(2) Any person who assists another person in voting by use of an mail-in absentee ballot or in-person absentee ballot shall complete the voter assistance form required by KRS § 117.255.

(3) The detachable flap on all mail-in absentee ballot envelopes shall have printed upon it the voter assistance form required by KRS § 117.255, notice of the penalty for failure to complete the form, and notice of the penalty under KRS § 117.0865.
(4) The State Board of Elections shall promulgate by administrative regulations under KRS Chapter 13A a voter assistance form which shall be in a form acceptable to the Attorney General.
