(1) The Secretary of State shall prescribe and furnish on request forms for: (a) An application for a certificate of existence;
(b) An application for a certificate of authority;
(c) An amended application for a certificate of authority; (d) A certificate of withdrawal;
(e) A change of registered office, registered agent, or both; (f) A change of principal address;
(g) The resignation of the registered agent, the registered office, or both; (h) An application for a reserved name;
(i) The renewal of a reserved name; (j) The transfer of a reserved name; (k) Name registration;
(l) The annual report; and
(m) An amendment to the annual report.
(2) The use of the forms referred to in paragraphs (e), (f), (g), (h), (i), (k), and (l) of subsection (1) of this section shall be mandatory. If the Secretary of State so requires, the use of some or all of the other forms listed in subsection (1) shall be mandatory.
(3) The Secretary of State may prescribe and furnish on request forms for other documents required or permitted to be filed by this chapter, but their use shall not be mandatory.

- Amendment: A proposal to alter the text of a pending bill or other measure by striking out some of it, by inserting new language, or both. Before an amendment becomes part of the measure, thelegislature must agree to it.
- Registered agent: means a registered agent appointed in accordance with KRS
14A. See Kentucky Statutes 14A.1-070 - Registered office: means the registered office identified in accordance with and
satisfying the requirements of KRS §. See Kentucky Statutes 14A.1-070 - State: when applied to a part of the United States, includes territories, outlying possessions, and the District of Columbia. See Kentucky Statutes 446.010
(c) An amended application for a certificate of authority; (d) A certificate of withdrawal;
(e) A change of registered office, registered agent, or both; (f) A change of principal address;
