(1) Before revoking, suspending, imposing probationary or supervisory conditions upon, imposing an administrative fine, issuing a written reprimand, or any combination of these actions regarding any licensee or permit holder under the provisions of KRS § 335.300 to KRS § 335.399, the board shall set the matter for hearing as provided by KRS Chapter 13B.
(2) After denying an application, refusing to renew a license or permit, or issuing a written admonishment regarding any applicant, licensee, or permit holder under the provisions of KRS § 335.300 to KRS § 335.399, the board shall set the matter for hearing upon written request filed by the applicant, licensee, or permit holder within thirty (30) days of the date of the letter advising of the denial, refusal, or admonishment.
