1. The work certificate shall show

(1) The age, sex, place and date of birth and place of residence of the child;

Terms Used In Missouri Laws 294.054

  • Child: an individual under sixteen years of age, unless otherwise specified. See Missouri Laws 294.011
  • Parent: a child's parent, legal custodian or guardian. See Missouri Laws 294.011
  • Place of residence: means the place where the family of any person permanently resides in this state, and the place where any person having no family generally lodges. See Missouri Laws 1.020

(2) The name and place of residence of the child’s parent, guardian or custodian;

(3) The name and address of the employer; and

(4) The nature of the employment for which the work certificate is issued.

2. Work certificates authorizing the employment of a child during nonschool hours shall be of a form and color distinct from those authorizing full time employment.
