‘Professional incompetence’ means lack of ability safely and skillfully to practice dentistry, or to practice one or more specified branches of dentistry, arising from:

- controlled substance: means a drug, immediate precursor or other substance which is listed in schedule I, II, III, IV or V for control by the State Board of Pharmacy pursuant to Nevada Revised Statutes 0.031
1. Lack of knowledge or training;
2. Impaired physical or mental capability of the dentist;
3. Indulgence in the use of alcohol or any controlled substance; or
4. Any other sole or contributing cause.
