Terms Used In New Hampshire Revised Statutes 290:1
- person: may extend and be applied to bodies corporate and politic as well as to individuals. See New Hampshire Revised Statutes 21:9
- state: when applied to different parts of the United States, may extend to and include the District of Columbia and the several territories, so called; and the words "United States" shall include said district and territories. See New Hampshire Revised Statutes 21:4
Whenever a person shall die, the physician, advanced practice registered nurse, or physician assistant attending at the last sickness shall complete and deliver to the funeral director, next-of-kin as defined in N.H. Rev. Stat. § 290:16, IV, or designated agent under N.H. Rev. Stat. § 290:17 or shall complete electronically and forward immediately to the division of vital records administration, a death record, duly signed, setting forth, as far as may be, the facts required by the department of state, division of vital records administration pursuant to N.H. Rev. Stat. § 5-C:63. The cause or causes of death shall be printed or typed on all records required to be furnished under this section. The funeral director, next-of-kin, or designated agent shall transmit electronically the record of death to the division of vital records administration and the certifying physician, advanced practice registered nurse, or physician assistant shall then electronically complete the cause of death information into the transmitted record.
